Seventy-three seconds after liftoff, Challenger broke apart. All seven astronauts aboard were killed, including Christa McAuliffe, a schoolteacher whose students were watching live.

The cause wasn't a freak accident or an unknowable risk. It was a decision — made by smart, experienced, well-meaning people in a meeting the night before. And it's one of the clearest examples we have of how groups talk themselves into catastrophe.

The warning

The shuttle's rocket boosters were sealed by rubber O-rings. Those seals had a known weakness: in cold weather they stiffened and could fail to seal.

The forecast for launch morning was unusually cold — far colder than any previous launch. The engineers at the company that built the boosters looked at the data and reached a clear conclusion: do not launch.

The dangerous comfort of a meeting where everyone nods

We tend to think a smooth meeting is a good meeting. Everyone aligned, no friction, decision made.

But the absence of conflict in a high-stakes decision isn't a sign of health. It's often a sign that something has gone quiet that shouldn't have.

The strongest teams build the opposite instinct. They get suspicious when everyone agrees too fast. They actively protect the person with the objection. They treat dissent as a gift, not a threat — because the alternative is a room full of people privately hoping someone else will speak up. That instinct has a structure, and it can be built deliberately.

The Challenger engineers spoke up. The system around them was built to make that not matter.

That's the real lesson. It isn't "listen to engineers." It's: the way a group makes decisions can quietly overpower what any individual in it actually knows.
